Key Responsibilities
  • Evaluate resident information to inform assessment and treatment processes.
  • Conduct comprehensive assessments to identify communicative deficits and equipment needs.
  • Design and implement personalized treatment plans with achievable goals.
  • Maintain accurate documentation and adhere to treatment guidelines.
  • Supervise aides and instruct them in appropriate procedures.
  • Document progress notes after each treatment session.
  • Adjust treatment programs based on resident performance.
  • Provide clear discharge summaries following treatment.
  • Educate residents and families about treatment procedures and home care.
  • Assist in referrals for additional services post-discharge.
  • Participate in rehabilitation team conferences regarding resident progress.
  • Engage in departmental meetings and quality improvement initiatives.
  • Access continuing education to maintain clinical expertise.
  • Participate in in-service education sessions.
  • Promote the services of HealthPro Heritage within the community.
  • Comply with regulatory and professional standards.
  • Uphold facility policies and participate in quality improvement programs.
  • Perform additional duties as required to ensure quality resident care.
  • Understand reimbursement processes and allocate resources effectively.
  • Demonstrate knowledge of relevant regulations and guidelines.
  • Exhibit a professional demeanor that fosters confidence in the rehabilitation program.
  • Work independently while recognizing when supervision is necessary.
  • Meet expected productivity levels for the position.

Qualifications

  • Master's Degree in Speech and Language Therapy.
  • Current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association.
  • Commitment to ongoing professional development and education.
  • Valid state licensure in Speech Language Pathology.
