Key Responsibilities:

  • Evaluate resident information to inform assessment and treatment processes.
  • Conduct comprehensive assessments to identify communicative deficits and equipment needs.
  • Design and implement individualized treatment plans with achievable goals.
  • Maintain accurate documentation and comply with treatment guidelines.
  • Supervise aides and instruct them in appropriate procedures.
  • Document progress after each treatment session and adjust treatment plans as necessary.
  • Orient residents to treatment services and address any potential risks.
  • Collaborate with nursing staff and families to enhance residents' functional abilities.
  • Educate residents and caregivers on therapeutic procedures for home continuation.
  • Assist in referrals for additional services post-discharge and stay informed about community resources.
  • Participate in rehabilitation team conferences to discuss resident progress.
  • Engage in department meetings and quality improvement initiatives.
  • Stay current with continuing education relevant to your discipline.
  • Contribute to in-service education sessions and quality improvement processes.
  • Promote HealthPro Heritage's programs and services within the community.
  • Adhere to regulatory and professional standards while maintaining facility policies.
  • Perform additional duties as required to ensure quality resident care.
  • Understand reimbursement processes and allocate therapy resources effectively.
  • Demonstrate knowledge of relevant regulations and guidelines.
  • Project a professional image to foster confidence in the rehabilitation program.
  • Work independently while recognizing when supervision is needed.
  • Meet expected productivity levels for your role.

Qualifications:

  • Master's Degree in Speech and Language Therapy.
  • Current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association.
  • Commitment to ongoing professional development and continuing education.
  • Valid state licensure in Speech Language Pathology.
