Overview

LHC Group is seeking a dedicated Clinical Liaison to enhance our outreach and partnerships for our Long-Term Acute Care (LTAC) facility.

This role is pivotal in establishing connections within the healthcare community to support patient transitions from various healthcare settings to our LTAC services. Ideal candidates are dynamic, proactive RNs, LPNs, or RTs looking to leverage their clinical expertise in a sales-oriented environment.

Key Responsibilities

The Clinical Liaison's primary function is to ensure a smooth transition for patients moving from a facility to LHC Group's LTAC for ongoing care. Responsibilities include:

  • Responding to patient referrals and conducting comprehensive pre-admission assessments.
  • Maintaining continuous communication with healthcare providers throughout the transition process.
  • Collaborating with the Administrator to identify target referral audiences and develop effective strategies for building referrals.
  • Engaging with various departments to facilitate the referral and evaluation process, addressing any barriers to admission.

Qualifications

Experience Requirements

Minimum of one (1) year of experience in an acute care hospital setting.

License Requirements

Possession of a valid driver's license and independent means of transportation is essential. Current RN, LPN/LVN, or RT license in Louisiana or applicable professional licenses is required.

Skill Requirements

Proficiency in computer applications, including spreadsheets and word processing. Strong organizational skills and effective personal communication abilities are necessary.

Desired Skills

Familiarity with the LTAC industry is preferred.
