Job Summary

U.S. Renal Care is seeking a highly skilled Dialysis Registered Nurse to join our team as a Home Hemodialysis Trainer. As a key member of our organization, you will be responsible for providing exceptional patient care and training to patients undergoing home hemodialysis or peritoneal dialysis.

Key Responsibilities
  • Train patients and their care partners in the practice of self-care dialysis, ensuring compliance with federal and state regulations.
  • Conduct home visits to assess the patient's home environment and evaluate their performance and home environment regularly.
  • Communicate patient assessment results, reassessment, and ongoing monitoring to physicians, team members, and other stakeholders as necessary.
  • Document all nursing services in the Electronic Medical Record, ensuring accurate reflection of patient status and nursing interventions.
  • Participate in infection control monitoring, implementation, and recording as requested.
  • Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in a timely manner.
  • Continuously seek to improve patient and quality outcomes through tracking and trending of program and patient performance.
Requirements
  • Knowledge of federal, state, local laws and regulations, including healthcare professionals' practice act requirements.
  • Ability to work independently and as part of a team to achieve exceptional patient outcomes.
  • Excellent communication and documentation skills.
  • Flexibility with staffing locations and hours to accommodate patient and USRC home program needs.
