Value-Based Care Management Registered Nurse

The Value-Based Care Management Registered Nurse plays a critical role in improving patient outcomes and reducing healthcare costs. This role involves interacting with patients, families, and support networks to provide comprehensive assistance in all areas related to ongoing education, medical history, medications, and lifestyle changes.



Key Responsibilities:

* Develop and implement value-based care clinical operations activities across the continuum of care.
* Provide health education and treatment decision support for patients with chronic kidney disease.
* Collaborate with patients, support networks, and interdisciplinary teams to assess patient needs and develop personalized care plans.
* Conduct in-person patient visits and outbound calls to assess patient health status and identify gaps or barriers in treatment plans.
* Coordinate with physicians and other healthcare professionals to support the quality and level of care provided for patients with chronic kidney disease.
* Assist with early identification and support for dialysis and transplant needs by working closely with the Nephrologist and Value-Based Care Team.

Requirements:

* Knowledge of federal, state, and local laws and regulations, including healthcare professionals practice act requirements.
* Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
* Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
