Job SummaryWe are seeking a skilled Home Care Nurse Specialist to join our team. As a key member of our healthcare team, you will be responsible for planning, organizing, and directing home care services. Your expertise in nursing, community health education, and patient care will be essential in delivering high-quality care to our patients.
Responsibilities- Conduct initial assessments of patients and families to determine home care needs
- Provide comprehensive physical assessments and histories of current and previous illnesses
- Regularly re-evaluate patient nursing needs and use health assessment data to determine nursing diagnoses
- Initiate and develop Plans of Care, which establish goals based on nursing diagnoses and incorporate therapeutic, preventative, and rehabilitative nursing care actions
- Counsel and involve patients and families in accomplishing healthcare goals, meeting nursing and related needs while promoting patient/family independence
- Communicate with physicians regarding patient needs and report any changes in patient condition
- Coordinate services and schedules with the Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC/Scheduler)
- Experience in nursing, community health education, or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong time management and organizational skills
- Knowledge of home health care services and procedures
The estimated salary for this position is $65,000 - $85,000 per year, depending on experience. We offer a competitive benefits package, including health insurance, paid time off, and professional development opportunities.
