About the Role:

As a Supervisor of Care Coordination at Callen-Lorde Community Health Center, you will play a critical role in overseeing a team of dedicated professionals and ensuring the delivery of high-quality care to our patients. You will be responsible for managing caseloads, providing clinical supervision, and coordinating care across various disciplines.

Key Responsibilities:

  • Team Leadership: Supervise a team of Patient Navigators, Case Managers, and Supportive Case Managers, providing guidance, support, and performance evaluations.
  • Case Management: Provide direct case management services, including intake, assessment, treatment planning, and ongoing reassessment.
  • Care Coordination: Coordinate care with medical providers, behavioral health professionals, and other community resources to ensure seamless patient care.
  • Entitlements and Referrals: Assist patients in accessing entitlements and referring them to appropriate services.
  • Documentation: Maintain accurate and timely documentation of all patient encounters and activities.
  • Performance Management: Monitor staff performance, provide feedback, and address any performance issues.
  • Quality Improvement: Participate in quality improvement initiatives and contribute to the development of best practices.
  • Professional Development: Attend conferences, trainings, and workshops to stay updated on the latest trends and best practices in care coordination.

Work Schedule: This is a full-time position with evening hours.
