Position Overview:

The Client Support Specialist plays a crucial role in delivering on-site case management services to eligible individuals. This position involves identifying client needs and coordinating psychosocial, rehabilitative, and medical services in collaboration with a multidisciplinary team.

Key Responsibilities:

  • Conduct comprehensive interviews with clients, family members, and service providers to gather necessary information for assessing program eligibility and case status.
  • Evaluate the psychosocial, economic, and physical needs of clients; continuously assess their support systems and available community resources to develop and implement tailored service plans.
  • Create and execute a case management plan that outlines intervention strategies to address client needs, including alternative housing options, vocational training, social support, healthcare services, legal assistance, and financial aid, adhering to regulatory guidelines.
  • Facilitate referrals to suitable community resources, including medical, emotional, transitional, and permanent housing programs, as well as job training initiatives.
  • Maintain accurate and thorough case documentation; prepare reports, correspondence, and other necessary written materials; input data into the Homeless Management Information System (HMIS).
  • Regularly review assigned cases to assess ongoing service needs and participate in case review meetings.
  • Engage in collaborative meetings with other community service providers to enhance service delivery.
  • Support additional client services, including seasonal programs and initiatives.

Qualifications:

Education: A Bachelor's degree in a social work-related field is preferred.

Experience: A minimum of two years of casework experience in a social service environment or relevant fieldwork is required; experience with SOAR is preferred.

Alternatively, any combination of education and experience that equips the candidate with the necessary knowledge, skills, and abilities will be considered.

Certifications: Candidates must obtain the Territorial Caseworker Certification within one year of employment.
