Position Overview:

The Patient Care Coordinator plays a vital role within the Patient Care Coordination Team, focusing on enhancing the health outcomes of our patients. This position involves utilizing reports from third-party payers alongside internally generated data to conduct patient outreach initiatives.

Key responsibilities include assisting patients with appointment scheduling, ensuring they receive appropriate follow-up instructions, and connecting them with necessary support service agencies. The Patient Care Coordinator will actively engage with physician practices to foster continuous quality improvement efforts, utilizing nationally recognized metrics such as HEDIS and evidence-based guidelines.

This role also entails providing essential communication and support to primary care providers regarding HEDIS measures, health maintenance, and the management of chronic conditions within their patient panels.

Job Requirements:

  • Completion of an accredited medical assisting program or graduation from a formal medical services training program of the United States Armed Forces, or possession of a Practical Nursing Degree.
  • A minimum of two years of experience in a clinical practice or healthcare environment.
  • If a Licensed Practical Nurse (LPN), a current Florida LPN License is required. If a Medical Assistant (MA), certification/registration with one of the following is necessary: American Association of Medical Assistants (AAMA), American Medical Technologist (AMT), NCMA National Certified Medical Assistant (NCCT), National Health Career Association (NHA), or AMCA-American Medical Certification Association certification. BLS certification through the American Heart Association is also required.
  • Strong interpersonal skills, ability to build consensus, and the capability to work independently. Excellent problem-solving abilities, clinical expertise, and proficiency in data-driven decision-making are essential. Candidates should possess outstanding communication skills and be proficient in Microsoft Word, Excel, and PowerPoint, along with demonstrated knowledge of EHR navigation. Familiarity with medical terminology, anatomy, and physiology is expected.
