Job Overview

The Licensed Practical Nurse is responsible for the assessment and management of patient care within the Screening & Prevention program.

Key Responsibilities
  • Manage incoming patient orders, conduct thorough chart evaluations, triage patient inquiries, and perform phone assessments to ascertain patient eligibility based on established screening protocols.
  • Act as a liaison between departments, ensuring effective communication with referring offices, physicians, and medical facilities regarding patient concerns and follow-up requirements.
  • Provide clinical guidance to patients and healthcare providers concerning procedures, preparation instructions, and eligibility criteria.
  • Review and relay preparation instructions to patients while submitting orders for procedural preparation under the supervision of physician management.
  • Analyze results from scheduled colonoscopy procedures, ensuring timely communication of findings to patients and referring providers, while maintaining accurate documentation in patient charts.
  • Administer treatments as prescribed by healthcare providers and respond promptly to patient prescription requests and inquiries.
  • Oversee patient recall for follow-up colonoscopies and adhere to directives from the performing physician.
  • Document observations, interventions, and evaluations related to patient care management accurately, utilizing advanced electronic medical record systems.
  • Ensure proper labeling and provision of insurance information for referrals to laboratories, imaging services, or specialists, adhering to insurance guidelines and maintaining accurate records of services rendered.
  • Facilitate patient education and discharge planning to enhance patient understanding and care continuity.
  • Maintain up-to-date knowledge, certification, and licensure in the field.
  • Commit to delivering high-quality service and patient care.
Qualifications

Education Requirements
  • Technical Diploma or Certificate in Practical Nursing is required.
Licenses and Certifications
  • Must possess a Licensed Practical Nurse license in Florida or an eligible compact state license upon hire.
  • BLS for Healthcare Providers certification from the American Heart Association is required upon hire.
Essential Skills and Competencies
  • Exceptional verbal and written communication skills.
  • Proficient understanding of patient care documentation and histories.
  • Strong knowledge of patient evaluation and triage processes.
  • Expertise in medication administration and patient care standards.
  • Familiarity with clinical operations and documentation practices.
  • Knowledge of inventory control and ordering of medical supplies and equipment.
  • Ability to read and comprehend the English language proficiently.
