Position OverviewWe are seeking a Social Services Coordinator (BSW) - Grief Support Specialist to become a vital part of our team.
This role will report directly to the Executive Director or Administrator and will focus on delivering psychosocial support and grief assistance to patients and their families.
- Conducts psychosocial evaluations, case management, educational initiatives, and ongoing assessments for patients and their families.
- Offers social work insights regarding modifications to the patient's care level.
- Manages casework and service coordination for patients needing information about or oversight of: family dynamics, the significance of illness to the patient and their family, cultural and spiritual perspectives related to illness, long-term care planning (including placement processes), and advanced care planning.
- Ensures the safety and well-being of the patient environment.
- Addresses the grief needs of families through home visits, phone interactions, group sessions, memorial services, correspondence, and other personalized support methods.
- Bachelor's Degree in Social Work from a CSWE accredited institution.
- Licensure as mandated by the state where the hospice operates.
- A minimum of one year of experience as a Social Worker in a healthcare or hospice environment is required; three to five years is preferred.
- Familiarity with the needs of terminally ill patients and their families, along with a comprehensive understanding of hospice care and the psychosocial aspects of illness, loss, and death; ability to apply knowledge of the unique requirements of hospice patients and families; sensitivity to the challenges posed by life and death issues faced by individuals with terminal illnesses; tactfulness in interactions with these patients and their families; capability to provide support during times of crisis; ability to manage the emotional toll of repeated loss.
