As a key member of our public health team, you will provide health education, case management services, and social and environmental assessments to assigned individuals and families.

Our team is dedicated to promoting the health and well-being of our community, and we are looking for a passionate and dedicated individual to help us achieve our mission.

Responsibilities:

* Provide health education and case management services to assigned individuals and families
* Conduct social and environmental assessments to identify areas of need and opportunity
* Develop and implement plans to address identified needs and opportunities
* Collaborate with community-based organizations and other stakeholders to promote health and well-being

Requirements:

* Bachelor's degree in Social Work, Early Childhood, Public Health, Human Services field, or other relevant field
* One year of home visiting experience working with pregnant women, infants, and children
* Knowledge of community resources and experience working with a variety of community-based organizations preferred
* Certified Lactation Consultant preferred
