Job Summary: The Recovery Vocational Case Manager is responsible for program participant recruitment, case management, employer engagement, engagement with recovery community, placement into competitive community employment, retention, and connection to wrap around supports. This position will work specifically with individuals in recovery from substance use disorder. Essential Functions: 
  • Participate in the development and implementation of case finding and client recruitment initiatives.
  • Act as case manager for assigned program participants and provide information about and referral to services needed through community resources (i.e.: adult education classes, childcare, transportation, credit counseling).
  • Implement training and interventions to further career development of assigned clients, including job readiness training, job seeking support, employment retention support, and barrier resolution.
  • Act as liaison between assigned program participants, employers and referral and funding sources.
  • Provide post-employment support and assist program participants to identify and utilize natural support systems.
  • Educate program participants, community organizations and employers concerning HGI programs and services.
  • Act as an advocate for assigned program participants.
  • Cooperate and maintain good rapport with program participants employers, staff and referring agencies.
  • Maintain all relevant program participant records, case notes and other documentation including attendance records and progress reports.
  • Complete and submit quarterly reports and program proposals to funding source(s) as directed.
  • Complete monthly progress reports and submit fee-for-service invoices as directed.
  • Assist with program evaluation and community needs assessment to identify program and service needs.
  • Participate in development and submission of program and service proposals to expand and diversify program funding as directed.
  • Participate in agency meetings and committees as directed.
