Faith-based family health care center seeking a full-time (40 hours per week) Behavioral Health Therapist to join our growing behavioral health team The Behavioral Health Therapist acts as an advocate and helps clients gain access to community-based resources and counsels patients on a 1:1 basis regarding their personal issues, such as mental illness, trauma, addiction and abuse. LCSW or LPC credentials are required.

**This position offers a retention bonus of $5,000 upon completion of the first year of tenure and $5,000 upon completion of the second year of tenure**

QUALIFICATIONS

  • MUST be an LCSW or LPC.
  • Experience working with patients who have suffered from trauma, abuse and chemical dependency.
  • Experience working with the healthcare needs of underserved populations.
  • Experience in a health care setting helpful.
  • Bilingual/Spanish speaking a plus.

KEY JOB RESPONSIBILITIES

  • Provides 1:1 psychosocial counseling for ELFHCC patients.
  • Complete a psychosocial assessment that includes social, economic, cultural, age-related factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems.
  • Coordinates and plans programs and activities to meet the patient's social and emotional needs.
  • Collaborates with other members of ELFHCC's behavioral health team to provide co-management activities for patients who require additional services.
  • Complete a psychosocial assessment that includes social, economic, cultural, age-related factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems.
  • Develops and implements a comprehensive, interdisciplinary plan of care which includes patient preferences. Establishes a plan for regular contact with each patient to review progress and assess the potential for additional needs,

Salary range commensurate with education and experience.
