Primary Function/General Purpose of Position Under the supervision of the Business Operations Manager, the Intake Coordinator is responsible for the administration of all incoming patients and any other administrative duties for the Hospice clients and staff. 4 10 hours shifts a week Employment Qualifications Education: High school diploma required some college preferred. Skills: Good typing, clerical and computer skills, customer service required . Experience: Three (3) year clerical experience in Hospice preferred; Two (2) to Three (3) years’ experience with insurance verification preferred. Strong organizational skills required and understanding of medical terminology preferred. Computer/data entry experience required. Knowledge of Medicare and Medicaid guidelines preferred. Interpersonal and telephone communication skills needed to relate effectively with a variety of people is required. Essential Job Functions Verify MDs information at the initiation of the referral (NPI, PECOS, licenses, demographics) and enter IT tickets as needed to update information Discharge patients in the Hospice system Answer phones Responsible for incoming calls with new referrals Coordinate with clinical staff and referral sources on all necessary documents to ensure accurate and timely insurance billing, incoming patient benefits and authorization. Responsible for Work queues as assigned Monitors fax machines for physician orders and request Responsible for ensuring all documents faxed are addressed timely Assesses patient information to determine eligibility for Hospice Services. Responsible for obtaining payor coverage for Hospice services and communicates coverage and benefit information to patients/customers, referral sources, Hospice managers, and staff assigned to the case. Communicates the status of referrals to referral sources, other organizations involved in the care of the patient, and appropriate Hospice managers. Provides community resource information to those persons not qualifying for Hospice services as appropriate. Begins the development of the preliminary patient plan of care by creating patient and medication profiles, discipline referrals, and Face to Face documentation per regulations and BSHSI policy. Communicates the details of patient information and preliminary care plans to the appropriate Hospice staff. Assists in coordinating the initiation of simultaneous services to be provided, to include but not limited to: home infusion services, private duty services, and home medical equipment. Documents all referral information and preliminary care plans in an accurate and timely manner Enhances professional growth and development through participation in continuing education programs and in-service sessions. Participates in performance improvement activities as indicated. Demonstrates knowledge of regulations and standards pertinent to Hospice services to include, but not limited to: Medicare, Medicaid, Private Insurances TJC, and state law and regulations. Screens referrals and patient status via electronic verification systems. Adheres to BSHSI rules, regulations, and policies. Performs other duties as assigned. Assists and supports other departments when needed. Corporate Responsibility Program Other Duties as assigned Other Job Functions Required to perform weekend Intake Coordinator Duties - every 3 to 4 weeks, may be required to work staggered hours as needed to meet the demands of the agency.
