Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team of hospice professionals. As a Hospice Nurse, you will work in private homes, residential care facilities, nursing centers, and other hospice care environments, providing comprehensive care to patients and their families.

Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
  • Develop and implement individualized Care Plans based on patient assessments in the EMR system within 48 hours
  • Administer medications and treatments as prescribed by physicians, and call medication into pharmacy
  • Monitor patients' condition and document changes in their health status
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Provide education and support to patients and their families on end-of-life care options
  • Assist with pain management and symptom control
  • Offer emotional support to patients and their loved ones
  • Attend IDG meeting and trainings
  • Must input and complete visit notes timely
  • Perform services in accordance with the physician's orders and the established plan of care
  • Ensure delivery of care is compliant with the federal and state regulations

Qualifications

  • Must be a Registered Nurse (RN), graduate of an approved school of clinical education and licensed in the state where they currently practice
  • Must have at least one year experience as a Registered Nurse
  • Must have 1+ year of hospice nursing experience
  • Medicare hospice experience is preferred
  • Must have basic demonstrated technology skills, including operation of a mobile device
  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider
