Licensed Social Worker - Home Care - Mercy Health Lorain
General Summary
The Social Worker coordinates all social services activities within the hospital for the pediatric, adolescent, adult, and geriatric patients. The Social Worker screens all geriatric and pediatric in-patient and observation admissions for discharge planning needs, follows state and federal regulations for Long Term Care medically related social services, screens for abuse or neglect and refers to appropriate agency or authority, provides assistance in seeking financial aid, collaborates with community agencies/resources in arranging specialized care and/or equipment, refers patient spiritual care requests to the Volunteer Chaplain Program, provides crisis intervention, and facilitates the adoption process. As well, the social worker promotes the institution’s philosophy, mission, and values during all interactions with patients, families, staff, and others.
Minimum Knowledge and Skills, Etc.

Bachelor’s degree in Social Work.

Current Social Work licensure from the state of Ohio.

Successful completion of general orientation and hospital probationary period.

Obtains/maintains a minimum of 12 contact hours of continuing education annually which includes Fire Safety and CPR Recertification.

Prior experience in a medical setting is preferred.

Interpersonal skills with the ability to deal effectively with multiple complex demand of relationships on the departmental level, as well as with internal and external customers.
