Job Summary

Key Responsibilities
  • Evaluate and assess the mental health and substance use-related symptoms of medical patients.
  • Intervene in crisis situations during business hours.
  • Provide brief assessments to clarify behavioral health diagnoses and to inform care plans.
  • Define targeted treatment goals with and for the patients.
  • Provide short-term counseling to an established schedule of Lorain County Health & Dentistry patients, which may also include family members.
  • Identify patients with more intensive behavioral health needs and assist these patients in connecting to appropriate care within the community.
  • Ensure all plans and strategies developed are personalized, age-specific, and culturally appropriate.
  • Provide education, counseling, and support to patients using trauma-informed and research-based interventions whenever possible.
  • Develop positive, professional relationships with patients.
  • Promote good health throughout the facility in conversation and behavior.
  • Accurately and concisely document all patient contact on a timely basis in medical records.
  • Collaborate with medical and psychiatric providers to ensure integrated care.
  • Participate in a Peer Review Process to ensure quality and consistency of BH service delivery.
  • Comply with HIPAA and 42 CFR guidelines at all times.
Requirements
  • Bachelor's Degree in Social Work, Counseling, or related field.
  • Valid license in the State of Ohio as a social worker (LSW) or Professional Counselor (LPC).
  • Minimum of two years of experience in a mental health treatment setting.
  • Experience in which behavioral health is integrated into primary care is preferred.
  • Ability to maintain a calm and professional demeanor and communicate enthusiastically with patients.
  • Ability to be responsive in working with a culturally diverse patient population.
  • Excellent communication skills with the ability to master a therapeutic communication approach.
  • Ability to independently coordinate multiple tasks.
  • Ability to work cooperatively with others.
  • Bilingual Spanish is preferred.
