Outreach Worker

CareSTL Health is seeking an Outreach Worker to serve as the organization's point of contact with the community and assist consumers with navigation and enrollment in various health plans. The Outreach Worker will develop and manage relationships with health plan service providers, manage monthly reporting, and facilitate health care and social service resources to support the health and well-being of the organization's clients and potential clients. In addition, this position also requires the ability to communicate effectively, educate, and network with people of diverse backgrounds and cultures.

ESSENTIAL FUNCTIONS:

  • Assist consumers with enrolling in health plan offerings and other program and service initiatives as needed
  • Track enrollment and outreach activities and outcomes, and provide monthly reports
  • Make outreach calls to health plan members
  • Bridge communications and information between health plan members and healthcare delivery systems
  • Ensure routine adherence to all regulations surrounding SA, CMS, MPCA, etc., as well as all laws and regulatory requirements surrounding confidentiality

OTHER FUNCTIONS:

  • Organize and attend community events and health fairs as assigned
  • Raise organizational service awareness in the community
  • Perform other skill-related duties as assigned

Education:

  • High School Diploma or GED Required

Experience:

  • 1-2 years of customer service experience required
  • Experience working in a clinical/medical environment is preferred

Transportation Requirement:

  • Must have Reliable Transportation and be personally insured
  • Must have a willingness to use reliable personal transportation in a work capacity
  • Must be able to travel up to 25%

Additional Information:

  • Clinic hours are Monday - Friday, 8 a.m. to 5 p.m.
  • 11 paid holidays per year.
