Social Workers (MSW) are responsible for the following:

  • In collaboration with the interdisciplinary team, provides medical social work acute care discharge planning services.
  • Advocates on behalf of patients/families to enable them to obtain necessary services including but not limited to post-acute facilities, hospice, dialysis, or linking them to community resources.
  • Participates in the promotion of quality outcomes, patient satisfaction, cost effective care delivery and safe transitions of care.

The Social Worker (MSW) will provide services related to the patientsrsquo; needs and work with the interdisciplinary team, patient/representative to create a discharge plan.

Qualifications for this position include:

  • Masters of Social Work (MSW)
  • Will consider BSW with Active NJ Certification
  • Two (2) years hospital based social work experience (preferred)
  • Active New Jersey LSW license (required)
