The Team Coordinator is a key liaison between patients and their Patient Centered Medical Home Team, including providers, clinic staff, and other health care professionals. Their primary function is to provide leadership for a seamless experience of care for patients calling their health care team, providing answers to patients' questions, or connecting the patient with another team member, tracking clinic quality measures and facilitating outreach to patients regarding services that are due.
