Phoenix, United States Select Specialty Hospital Full time
Overview

Critical Illness Recovery Hospital

Clinical Liaison (RN, RT or LPN)

Salary range: $75,000/year - $95,000/year (based on experience), plus monthly bonuses

Are you looking for ways to advance your career beyond bedside care? Clinical Liaisons at Select Specialty Hospital assist with transitioning patients from critical care units in local hospitals into our critical illness recovery hospitas in Phoenix. You will not provide direct patient care, but you will conduct clinical assessments of patients and help referring case managers gain the appropriate approvals to discharge patients into the next, best level of care for their recovery. This role will train you in marketing and case management skills. It will allow you to work with multiple Phoenix area hospitals, and consequently widen your scope of understanding of the wider healthcare community.

Responsibilities

A Clinical Liaison serves as an advocate for patients during the pre-admission process. Liaisons work with all clinical disciplines in evaluating current and potential patients' statuses and medical needs. You will serve as a subject matter expert in discharge planning, insurance approval, and critical illness recovery for referring hospitals, their clinical teams and their patients.

Select's hospitals offer a post-ICU plan of care that focuses on mobility, weight-bearing, ambulation and nutrition - all crucial contributors to getting patients back to their baseline of health. Clinical Liaisons help hospitals in their local communities to identify post-ICU patients who have entered into a state of chronic critical illness and will benefit from this specialized extension of acute care.

  • Armed with a strong clinical background, payer knowledge and problem-solving skills, you’ll work with physicians, discharge planners and patient's families to plan the best continuum of care.
  • Perform clinical assessments of referred patients, then counsel with patient's families to assist them in making the best care decisions for their loved one.
  • Partner with hospital case managers to create smooth patient transfer processes and help remove payer-related and operational barriers to getting appropriate patients into this highly effective level of care.

This is a salaried position that works Monday-Friday with periodic on-call responsibility.
