POSITION SUMMARY The Community Health Outreach Coordinator engages the community in the services and activities of the Health & Wellness Center and provides education, outreach and support to the community as well as individuals and families served. Maintains records of coordination of care, outreach, patient support and/or case management activities for reporting and tracking purposes and completes all documentation for client and program records in accordance with documentation timelines and in compliance with Medicaid, licensing and other regulatory requirements/agencies as defined by program. Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate. May involve visiting clients in their homes and/or community settings Attends and is prepared for scheduled supervision, team meetings, staff meetings or rounds. Seeks additional supervision or consultation as needed and follows through with supervisory directives. Demonstrates clear, concise, professional writing and oral reporting skills. Works effectively as a member of a multi-disciplinary team by communicating appropriately and positively with colleagues, families, internal and external service providers, administrative staff and actively preparing for and participating in team meetings.
