Eddy SeniorCare is a Program of All Inclusive Care for the Elderly (PACE) which provides support to frail seniors struggling to maintain themselves in the community setting. The program is built upon strong communication between the interdisciplinary departments which include: medicine, nursing, therapy, social work, nutrition, homecare, day center services and transportation. Essential Duties and Responsibilities Include: Assesses the patient, family and home environment for areas where social work planning and interventions can be of assistance. Participates in pre-admission screenings of patients referred for admission and attends all admission interdisciplinary team meetings. Determines patient and family needs related to social supports, financial support or counseling, housing appropriateness and psychological supports. Confers with the patient and family regarding individual expectation and long and short term goals. Investigates the need for and makes arrangements for appropriate discharge plan when disenrollment is being pursued. Assists the patient or family in making necessary applications for public assistance, housing placement, pre-admission screening, etc. Intervenes in crisis, attending to the emergent needs of the patient and/or family relating to illness, disability, deterioration of independence, etc. Oversees, supervises and evaluates all Social Work staff. Assists in setting priorities of care and most effective use of staff time. Supervises Case Workers and MSW students as appropriate. Minimum Qualifications: Master’s degree in Social Work is required. NYS LMSW is required, LCSW preferred. Must have at least one year of previous experience working with the frail or elderly population. Additional experience in home care preferred. Pay Range: $33.50 - $45.68 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
