Key Responsibilities- Conduct thorough psycho-social assessments to identify patient needs and develop individualized treatment plans.
- Participate in treatment team planning and coordination to ensure comprehensive care.
- Coordinate discharge planning to ensure seamless transitions to post-hospitalization care.
- Provide therapeutic interventions and support to patients and their families.
- Collaborate with multidisciplinary teams to address clinical and administrative issues.
- Maintain accurate and timely documentation of patient care.
- Contribute to program improvement initiatives and quality assurance activities.
A Master's degree in Social Work is preferred, but a Bachelor's degree with extensive experience working with individuals with acute mental illness may also be considered. CSW licensure is required, and a closely allied field may be acceptable with relevant experience and qualifications.
Salary Range: $24.75-$36.53/hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
