Director, Case Management

The Director of Case Managementis responsible for planning, directing, and managing of the activities of theCase Management Program for North Shore Medical Center "NSMC". Theresponsibilities include the development and supervision of staff (RNs, SWs andnon-clinical support), evaluation and maintenance of case management skills andoutcomes, management of all policies and procedures, and ensuring adherence tothe key model components that drive outcomes. Customers include patients, patient's families, visitors, nursing staff,physicians, interdisciplinary team members and accrediting bodies. Works collaboratively with themultidisciplinary team to assure quality, timely and cost efficient care.
Qualifications
Required:
- Must have strong analyticalabilities to strategically plan, consensus build and problem solve.Demonstrates appropriate judgment in decision making. Must be able to communicate and convey complex information in aprofessional and courteous manner. Must have administrative skills to superviseand develop staff, plan, organize, direct, and evaluate.Must have proficiencyin data analytics. Must have strong understanding of clinical and financialoutcomes. Must have strong interpersonal skills. Must have ability to interact effectivelywith a wide range of personalities, ages, and diverse cultural backgrounds.Demonstrates appropriate communication skills for the patient population servedand other NSMC customers. Must be able to work in a fast paced environment anddemonstrate performance agility in a continuously changing environment.Demonstrates leadership in the change process.Good visual, verbal, hearingskills. Fluent in English. Proficiency in using computers and ability toquickly gain competency in various software applications, including hospital specific applications, MicrosoftOffice, cell phone, pager.
- Minimum 7 years acuteclinical experience.
- RN withMaster's Degree
- CurrentLicense as Registered Nurse in the Commonwealth of Massachusetts
Preferred:
Minimum 5+years of progressive management experience with demonstrated goal orientedoutcomes. Must have demonstrated leadership and change management experience.CaseManagement experience preferred.
