Job Summary and ResponsibilitiesThe Case Manager (CM) is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. The CM provides, coordinates, and directs care tailored to the needs of each rehabilitation patient. The CM collaborates with the Rehabilitation Program Director and Facility Case Management Director regarding departmental operations.
Key Responsibilities:
- Conduct program orientation for patients, families, and caregivers, covering the case manager's role, rehabilitation philosophy, and criteria for continued stay and discharge. This includes information on Medicare and insurance benefits, grievance procedures, treatment planning, and patient rights and responsibilities.
- Serve as the coordinator for patient, family, and caregiver education, promoting their involvement in team discussions regarding plans, goals, and progress through Family Conferences and other interactions.
- Ensure the implementation of the patient's treatment plan that aligns with their strengths, abilities, needs, and preferences, facilitating patient involvement throughout the rehabilitation process.
- Document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating the social, financial, or discharge needs and preferences of the patient, family, and caregivers.
- Promote consistent, positive patient interactions that advance the agenda of exceptional patient service.
