Overview: Enlightened, Inc. is seeking to hire a team of experienced Community Outreach Specialists to support our healthcare client. The COS will support and work on health promotion programs including but not limited to health fairs, community events, targeted outreach and/or care management programs. The ideal candidate will have 2+ years of work experience in healthcare, social work, or human services.

DUTIES AND RESPONSIBILITIES: Under the supervision of the Community Outreach Services Team Supervisor, the Community Outreach Specialist will

  • Interface with members in person, on the phone and through correspondence as appropriate to communicate key care management, benefit administration and health promotion messages.
  • Collaborate with the medical management/Integrated HealthCare Management (ICHM) team to identify and potentially identify solutions to address environmental factors and social determinants that may be impacting members.
REQUIRED SKILLS:
  • One to three years of health care, social work, or human service experience or equivalent.
  • Knowledge about the assigned community, specific health, and social issues.
