A Brooklyn-based healthcare company is actively seeking an experienced Director of Patient Coordination. In this role, the Director would oversee patient coordination and scheduling as well as oversee quality improvement initiatives to ensure quality of service.

Responsibilities:

The Director of Patient Coordination will:

  • Oversee team leaders to ensure their teams are set up for continued growth and quality of care for patients.
  • Ensure metric quotas for staff are met.
  • Direct all aspects of patient staffing and coordination.
  • Oversee all staffing daily summaries, case by case oversight and involvement with patient as needed.
  • Run reports to ensure staffing is accurate, caregivers have been assigned, and patients have been notified.
  • Accept or deny incoming cases based on diagnosis and caregiver availability.
  • Other duties as needed.

Qualifications:
  • 5+ years in home care
  • Experience overseeing a team
  • Knowledge of licensed home healthcare rules and regulation and familiarity with NYS DOH regulations
  • Knowledge of documentation and compliance guidelines
Desired Skills:
  • Ability to build rapport with clinical and non-clinical staff members.
