Position Overview:

The Physician Affiliate Group of New York (PAGNY) is seeking to enhance its team with a dedicated Patient Safety Coordinator. This role is integral to our commitment to providing exceptional healthcare services across our facilities. The coordinator will be positioned within a state-of-the-art healthcare environment that emphasizes safety and quality in patient care.

Key Responsibilities:
The Patient Safety Coordinator will play a vital role in the administration team, focusing on the following:

  • Incident Reporting: Monitor and manage safety event reporting, including near misses and commendable actions; develop and sustain a recognition program for positive safety practices.
  • Root Cause Analysis: Lead discussions on root cause analyses and engage in performance improvement initiatives for corrective actions.
  • Data Coordination: Oversee data collection and reporting for national safety initiatives, collaborating with quality departments to identify areas for improvement.
  • Safety Culture Assessment: Organize biennial surveys to gauge safety culture, analyze results by department, and present findings to leadership and staff.
  • National Safety Standards: Regularly track and report on national patient safety goals, utilizing standardized analytical tools to present trends and improvements.
  • Safety Meetings: Participate in system-wide safety council meetings and facilitate regular safety discussions at the facility level.
  • Leadership Rounding: Engage in executive rounds to address patient safety concerns and share improvements with frontline staff.
  • Education Initiatives: Conduct training sessions on safety protocols and the importance of reporting safety events.
  • Project Sponsorship: Mentor and support patient safety projects, encouraging participation in relevant conferences and award submissions.

Qualifications:
Candidates should possess one of the following:

  • Certification in Safety or Quality (e.g., CPPS) preferred; if not obtained prior to hire, completion within two years is required.
  • Proficiency in EPIC for data analysis and reporting.
  • Strong skills in Microsoft Office applications.
  • A valid New York State license as a Registered Professional Nurse with two years of relevant clinical or administrative experience.

Alternatively, candidates may have:

  • A Doctoral degree in Medicine, Nursing, Public Health, or Administration with one year of experience.
  • A Master's degree in a related field with three years of experience.
  • A Bachelor's degree with five years of experience.
  • A satisfactory combination of education and experience.
