Patient Engagement and Support Coordinator
The Patient Engagement and Support Coordinator plays a crucial role in facilitating outreach, intake, assessments, screenings, and comprehensive case management for individuals living with HIV who have not yet achieved viral suppression. This position reports directly to the Program Manager of the Retention and Adherence Program.
Key Responsibilities:
- Conduct outreach initiatives alongside Peer Navigators to connect patients with necessary care or to re-engage them in ongoing treatment.
- Engage with newly diagnosed individuals referred from various healthcare sources.
- Manage a caseload of patients by performing assessments, maintaining case notes, and developing service plans to monitor progress.
- Assist patients in securing essential insurance coverage.
- Participate in case discussions to evaluate patient progress and identify barriers to achieving viral suppression.
- Collaborate with Peer Educators to formulate tailored care plans for patients.
- Conduct home visits to reconnect with patients who have disengaged from care.
- Document all patient interactions and outreach attempts in the electronic medical record system.
- Attend relevant forums and conferences related to HIV/AIDS to remain updated on industry developments.
- Provide referrals to appropriate service providers as necessary.
- Engage in quality assurance and performance enhancement activities.
- Perform additional duties as assigned.
Qualifications:
- High School diploma or GED is required.
- Bachelor's Degree in Human Services or a related field is preferred.
- 1-5 years of experience in HIV prevention, treatment, and adherence support.
- Proficient in Microsoft Office Suite.
- Strong verbal and written communication skills in English are essential.
- Excellent interpersonal skills with the ability to employ calming techniques to de-escalate crisis situations.
- Bilingual candidates are preferred.
Physical Requirements:
- May require prolonged periods of standing, walking, sitting, and bending.
- Some repetitive motions may be involved in the execution of job duties.
