Pathways to Housing's Outreach staff works to build trust with those experiencing homelessness first by helping to meet some of their basic needs (including food, clothing, showers, laundry, transportation, identification, etc.). Then, they partner with each individual to access housing, employment, medical care, mental health care, substance abuse treatment, and other services.

The Outreach Specialists have the unique role of providing recovery services, health and wellness self management education, as directed by the Program Manager. The Peer Outreach Specialist is responsible for providing interventions to promote wellness and recovery to consumers, including addressing chronic and mental health conditions, physical activity and nutrition.

Approximately 85% of time will be spent in the community, directly providing services to persons living on the streets or in shelter. Candidates must be comfortable working in diverse settings, including shelters, streets, jails, hospitals, encampments, etc. While much of the work is independent and self-directed, there is also a strong team dynamic. Shift schedules will generally include hours between 7am-7pm, Monday through Friday, but schedules will be flexible to accommodate the needs of the community and may include early morning or overnight shifts. Successful candidates are committed to ending homelessness and have a fundamental belief that hope and recovery are always possible.

Please note the following:

  1. Hired Outreach Specialists provide proactive customer support through weekly visits to 15 retail stores throughout the district; offer referrals or provide services for those who need medical care, mental health care, housing services and referrals to community resources; provide reactive customer support to an additional 20-40 stores.
  2. The Outreach Specialists on this team are required to travel to multiple locations throughout the day, every day. Travel between sites will be at minimum 3 locations and will not exceed 6 per day within the Washington, DC area. Your travel between sites will be funded.
  3. This position requires work in close collaboration with other agencies and vendors, the other Outreach and DDSC Pathways team, as well as the program and executive directors. The selected applicant will play a key role in nurturing the ethos of presence, attentive care and accompaniment that is at the heart of Pathways to Housing DC’s service to adults experiencing homelessness.

Responsibilities Include:
  • Identify and engage individuals experiencing homelessness in a geographically assigned area and assess needs;
  • Proactively initiate non-judgmental conversation and a consistent presence; offer clothing, hygiene articles, and other basic items;
  • Work with a core group of individuals over time to develop a collaborative goal plan to meet needs, particularly those who may not independently seek services;
  • Serve as a bridge to health services assisting consumers in obtaining information about their illnesses and in building skills to better manage them;
  • Provide in-person assistance, including escorts and facilitating smooth transition (“warm handoffs”) to other service and housing providers;
  • Participate in weekly and/or situational supervisory sessions with Program Manager to review and plan weekly schedule, review note corrections and attend to other issues as needed
  • Participate in regular and/or situational supervisory sessions with Program Manager and Director of Homeless Services to review consumer health goals/progress, assess intervention techniques and further develop skills in health education, staff supervision and training.
  • Communicate consistently and effectively with Program Manager
  • Assist consumers in creating personal health plans;
  • Assist consumers in articulating and attaining personal health and recovery goals and assist the consumer in overcoming barriers to implementing the individualized plan;
  • Identify and pursue relationships with neighborhood health and wellness resources to further increase opportunities for social support; Identify other community resources and provide in-depth referrals to housing and social services;
  • Perform other related work as directed i.e. documentation in electronic medical record, administrative work, scheduling, regular communication with team members.
Qualifications:
  • HS Diploma/GED required; college degree in social sciences preferred
  • At least one year previous work experience in mental health program and experience working with others
  • Cultural sensitivity
  • Ability to communicate effectively both verbally and in writing
  • Comfortable operating computers, inputting information into a database, and documenting case notes
  • Advocacy experience or demonstrated mentor or leadership ability
  • Ability to maintain boundaries but comfortable using self and personal experiences as applicable
  • Lived experience of homelessness preferred
