Position Summary:

The Outreach Specialist is responsible for engaging with Veterans and their families across various regions. This role involves conducting outreach efforts to identify eligible veteran households for Supportive Services for Veteran Families (SSVF), particularly those who may be challenging to locate or assist. The specialist will assess program eligibility and perform necessary evaluations to determine prioritization within the Coordinated Entry system, ensuring accurate data entry into the Homeless Management Information System (HMIS).

Key Responsibilities:

  • Establish contact and build rapport with homeless and at-risk Veteran families, providing appropriate community referrals.
  • Conduct street outreach initiatives.
  • Focus on the most vulnerable Veteran families who are difficult to reach.
  • Perform assessments with Veteran families in need of services and document findings in HMIS.
  • Foster and maintain relationships with local social service agencies, shelters, and community organizations.
  • Collaborate with local VA facilities, including Vet Centers and Veterans Benefit Administration offices.
  • Participate in community events and initiatives.
  • Engage in Local Coordinated Entry processes.
  • Promote the services offered through the SSVF grant.
  • Attend multidisciplinary meetings and receive regular supervision.
  • Document outreach efforts meticulously.
  • Engage in professional development opportunities.
  • Assist fellow specialists as needed.
  • Fulfill additional duties as assigned.

Qualifications:

  • High School Diploma/GED with a minimum of two years of relevant experience preferred.
  • Bachelor's degree with at least one year of experience in a related field preferred.
  • Valid driver's license and capability to operate a vehicle.
  • Experience working with Veterans and understanding their unique challenges.
  • Proven outreach experience with local organizations and government agencies.
  • Strong written and verbal communication skills.
  • Able to thrive in a culturally diverse environment.
  • Commitment to the mission, including a focus on racial equity and social justice.
  • Ability to work effectively under pressure and as part of a team.

Working Conditions:

This role may require spending up to 70% of the time in the field, engaging with participants, traveling, and working outdoors or in shelters. Administrative tasks will be conducted in an office setting.
