Position Overview

The Advocate will lead the implementation of all grant activities, including but not limited to the following activities to take place at supportive living sites: expanding partners with Project EATS and building new partnerships with local farmers to establish farmers markets and CSA pick-up sites; helping residents establish community gardens, rooftop gardens, and/or window gardens as space and safety permit; offering healthy cooking and healthy eating classes; offering community meals and taste test events; educating residents on how to utilize SNAP benefits, FNMP, NYC Department of Mental Hygiene Health Bucks, and health insurance incentives to purchase food at farmers markets and through CSAs; offering free-to-resident CSA trials through the grant; creating and offering a monthly calendar of physical activities; etc. The Advocate will also ensure adherence to the timeline and budget, lead data collection, build partnerships, and complete grant reporting. The selected Advocate will hold bachelor's degrees in content areas relevant to the work or equivalent years of relevant work experience, have knowledge of healthy foods programs and safety net programs at a minimum. The ideal candidate will also have nutrition or agriculture education teaching or community education experience and/or a nutrition credential and/or knowledge of and relationships with local farmers.

Specific responsibilities will include:

  • Meet with clients, who screen positive for food insecurity, in person or utilizing telehealth, at our partner community-based health centers to discuss healthy food options/meal plans based on their health conditions and food access, order and distribute food, and linkages to community resources.
  • Screens patients for nutritional risk, ensures the efficient flow of the assessment and education processes and services meet all regulatory requirements.
  • Meets with clients weekly or bi-weekly, to ensure they have access to food and offer additional nutritional counseling and community resources for program participants.
  • Design, develop, and present relevant and appropriate nutrition education/messaging/ workshops.
  • Plan, conduct and participate in health activities for the programs and participants.
  • Provides nutrition counseling; nutrition behavioral therapy; lactation counseling; nutrition, physical activity, exercise counseling, and health education as components of nutrition program.
  • Coordinates the nutritional care needs of the participants by conferring with other healthcare providers and referrals to appropriate program and nutrition resources.
  • Offers nutrition and cooking demonstration workshops at least twice per month to participants.
  • Supports program participants' setting and making progress towards nutrition goals and track high-risk participants' progress in improving their health.
  • Helps to set up and maintain an onsite, health-conscious food pantry to ensure community stability.
  • Provides education and nutrition resources to the community about healthy lifestyles changes and positive nutritional outcomes.
  • Plans and attends Farmer's Market trips, distributes Health Bucks and maintains documentation.
  • Assists and coordinates health and nutritional education lesson plans for children and families.
  • Supports the highest possible patient satisfaction levels that ensure program effectiveness in attaining food and nutritional service goals for program participants.
  • Advocate on behalf of patients and families regarding their nutritional well-being.
  • Communicates and responds in a manner that consistently demonstrates respect and concern.
  • Contributes and assists in the preparation of reports for agency, department, and grant funders.
  • Monitors health, nutrition safety, and service records, including data and documents all activities.
  • Communicates and work with diverse families and professionals and guide staff in obtaining positive nutrition, health and safety, medical and dental results.
  • Participates in food and nutrition related research to demonstrate food and nutrition related outcomes and to develop food and nutrition policy and recommendations. Research and track food resources, including nutrition education programs, and other benefits/supports.
  • Maintain an ongoing collaboration between programs and other agencies, as well as funding partners to cultivate and maintain a network of providers who offer services and enhance programs.
  • Maintains relationships with and collaborate and communicate regularly with internal and external stakeholders regarding referral processes and program updates related to resource referrals.
  • Functions as a vital part of the Interdisciplinary team and performs as a liaison of the Supportive Housing and Services department.
Requirements
  • Bachelor's degree in food and nutrition, or related field.
  • Minimum one (1) year clinical nutrition experience preferred.
  • Minimum of two years of some training in food preparation, cooking, and safety and sanitation.
  • Experience in a kitchen environment is required. Food/safety certification preferred.
  • Demonstrable understanding of client-centered services and the principles and practice of Housing First Model, Harm Reduction Practices and Trauma-Informed Care.
  • Experience working with homeless and/or at-risk populations preferred.
  • Ability to exercise discretion in handling confidential information and materials.
  • Excellent interpersonal and verbal communication skills and organizational skills.
  • Computer literacy, particularly with Microsoft Office applications.
