Job Summary

HeartShare Human Services of New York is seeking a highly skilled and experienced Child Advocate Supervisor, RN to join our team. As a key member of our leadership team, you will provide administrative direction, professional and medical standards/advocacy to our Specialized Family Foster Care (SFFC) program.

Key Responsibilities

  • Supervise a team of up to three Child Advocates, ensuring quality and timely service delivery to families and children.
  • Review medically fragile referrals, placement capacity, and oversee the implementation of quality and timely service delivery.
  • Collaborate with the VP of Home Finding to develop and implement strategies for Medically Fragile bed development.
  • Ensure compliance with regulatory standards and requirements, including timely completion of Medical Necessity forms and scheduling of initial medical, dental, and behavioral health assessments.
  • Provide weekly and monthly group supervision, development, and evaluation of Child Advocates.
  • Coordinate closely with HSVS's 29-I clinic, Case Planning, and Children's Health Homes teams to provide comprehensive services and establish treatment goals.
  • Review medical treatment summaries and determine whether further treatment is recommended, including referrals and medications.
  • Provide training for foster children and staff on medical conditions, health education, medication administration, and safety precautions.
  • Approve and monitor vacations, absences, work schedules, and attendance records for staff.
  • Support the completion of adolescent assessments, sexual/reproductive health, gender identity, sexual orientation, and family planning consultation.
  • Attend/participate in trainings, case conferences, staff hiring, and engage in ongoing review of departmental policies, procedures, and practices to make recommendations for quality improvement.

Qualifications

  • At minimum a Bachelor's Degree required.
  • NYS Registered Nurse (RN) License required.
  • At least two (2) years of Supervisory Experience in Care/Case Management working with families and children.
  • At minimum three (3) years case management experience in a healthcare environment and/or in the child welfare system.
  • Ability to generate, interpret, and analyze data from multiple sources.
  • Up to 50% travel required, including visits to client's homes and assessments in the Children's Center.
  • Excellent organizational and time management skills with the ability to manage multiple tasks and requests simultaneously.
  • Establish and maintain respectful professional boundaries with individuals and families.
  • Good understanding of medical procedures and medical terminology.
  • Experience documenting in Electronic Health Records (EHR), NextGen EHR experience a plus.
  • Excellent written, oral, and interpersonal communication skills, and be a collaborative team player.
