  • Collaborate effectively with Graham Windham, case planners, primary health care providers, and birth and foster parents to develop comprehensive health plans for our children.
  • Conduct thorough initial health and risk assessments, gather detailed health histories, and obtain necessary consents from families.
  • Utilize various Electronic Health Record (EHR) systems to schedule appointments, upload medical information, and track patient progress effectively.
  • Manage appointment schedules and provide concise health summaries to case planners in advance of Family Assessment and Service Plan deadlines.
  • Ensure strict adherence to all applicable county, state, and federal regulations governing healthcare services.
  • Actively participate in High-Risk Meetings to ensure the delivery of optimal care for children identified as high-risk on your caseload.

Qualifications:

  • Licensed Practical Nurse (LPN) with a demonstrated commitment to providing exceptional care coordination, preferably within a pediatric setting.
  • Proficiency in Microsoft Office Suite and familiarity with NYS Connections system or other EHR platforms are highly desirable.
  • Exceptional communication, time management, and organizational skills are essential for success in this role.
  • Bilingual Spanish/English fluency is a significant asset.
