As a Manager of Clinical Programs you will : Lead a collaborative multidisciplinary health care team (which includes community health workers, nurses, care coordination specialists, and licensed clinical social workers). Be responsible for oversight of daily care team functions, coordination/assignment of caseloads, contract/regulatory compliance for the model of care, continuous quality improvement initiatives, and for assuring optimal productivity and efficiency without compromising the quality of work. Under the direction of Navvis' VP Enterprise Ambulatory Operation and via dotted line reporting to the SVP Medical Director of Hawaii, offers subject matter expertise, provides guidance and support, and supervises all care team staff Be responsible for ongoing one-to-one meetings with the staff, individual development plans and performance reviews, corrective action as needed, and maintaining applicable documentation Establish accountability with staff to meet performance benchmarks, monitors operational and financial reports and metrics, conducts (or provides oversight) for monthly case audits, and implement performance improvement process as necessary Provide ongoing education and competence measurement for care team staff Provide oversight to assure all staff are adherent to policies, procedures, and regulatory standards, and to assure all tasks and paperwork are submitted within timeframe defined by standard operating procedures. Coordinate or provide oversight for the coordination and documentation of daily huddles, interdisciplinary team meetings, and PCP huddles Ensures the ongoing development and implementation of care management policies and procedures Collaborate effectively with internal and external stakeholders to ensure the effectiveness and efficiency of the programs Serves as liaison with HMSA departments. Remain up to date and communicate any changes in HMSA programs and benefits. Maintain documentation relative to the activities of the department and prepare reports as necessary, including those related to Quality Improvement activities. Keeps designated management aware of progress toward goals and productivity. Other essential duties as assigned.

A Day in the Life: The colleague will work in a HIPAA compliant work-from-home environment. Occasional extended hours may be necessary to meet tight deadlines, and the ability to work under time constraints is valued. Adaptability to various work scenarios and flexibility in adjusting schedules based on operational requirements are important attributes for the role.

Requirements

Current unrestricted Hawaii RN license (Will consider candidates who can obtain a Hawaii RN license very quickly) 1-3 years of clinical experience 3-5 years of management/leadership experience Current valid drivers' license, and access to insured vehicle
