Essential Duties and ResponsibilitiesMarket analysis, along with competitive landscape, in which to differentiate our HC services to external referral sourcesMeet and exceed agreed upon F2F and virtual meeting goals with key decision makersThoroughly understand eligibility and enrollment process of the HarmonyCares suite of businesses in a given marketTrack performance in CRM demonstrating plan and adherence to budgeted targets for a given regionSchedule face to face or phone discussions with patients, POAs, or guardians to provide guidance on patient eligibility for all programsProvide direction on benefit enrollment program and processesObtain Voluntary Alignment Form from patients, POAs, or guardians at all timesAnswer inbound calls from patients, families, and facility staff members regarding various eligible programsPromptly, accurately, professionally triage all patient concerns related to care needs, scheduling and billing to the appropriate departmentOversee and document outreach calls conducted and meetings scheduled with external sourcesEnsure scheduled meetings with patients and their caregivers are attended timelyAdhere to company policies and procedures related to patient outreach callsComplies with PHI and HIPPA guidelinesMaintains consistent communication with Director- Enterprise Sales, Practice Managers, Hospice/HH Administrators, and local BU support staffPerforms other additional duties as assigned QualificationsREQUIRED Knowledge, Skills and ExperienceHigh school diploma or GEDSix months or more of administrative office experience or outside sales experienceMust maintain a valid driver’s license and good driving recordExcellent written and verbal communication skillsAbility to work independently and complete multiple tasksStrong organizational skillsComputer skills including but not limited to Word, Excel, Outlook, Report softwareDemonstrable success in quickly building telephonic relationships with eligible patientsPreferred Knowledge, Skills and ExperienceSelf-motivated and driven individual who does not require significant oversight to perform at a high level after initial orientation and training
