Job Summary

We are seeking a skilled RN Rehabilitation Community Liaison to join our team at Touro Infirmary. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and promoting a culture of excellence.

Key Responsibilities:
  • Assess, plan, implement, and evaluate patient care based on standards of care and policies of LCMC Health.
  • Delegate to professional and non-professional personnel involved in the delivery of care.
  • Facilitate and maintain a safe environment for patients and others with a specialty in wound care treatment and prevention.
  • Utilize the nursing process to deliver patient care, organizing, prioritizing, and coordinating all aspects of care for each assigned patient.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Collaborate with interdisciplinary teams to identify actions to improve patient care and education.
Requirements:
  • Basic Life Support/CPR certification.
  • Advanced Cardiac Life Support certification.
  • Graduate of an accredited registered nursing program.
  • Preferred: BSN and 1 year of experience in a related clinical area.
