Pierce County's first Certified Community Behavioral Health Clinic; Comprehensive Life Resources is looking for a qualified Care Coordinator position to join our team in downtown Tacoma Washington.

The Program for Assertive Community Treatment (PACT) is an evidence-based model of team-based care. PACT is comprised of Mental Health Professionals, Case Managers, Nursing, Peers, Psychiatric ARNPs, and Vocational Counseling.

Function: As the Care Coordinator, you play a crucial role on the front line of the PACT team, working in partnership with our referral partners to ensure the best care for our participants. This includes collaboration with inpatient psychiatric facilities, hospitals, and other behavioral health agencies. The Care Coordinator gathers collateral information for new referrals and sets up interviews with the clinical care team. The Care Coordinator is responsible for answering client calls and using trauma-informed language when troubleshooting and triaging to appropriate staff.

The Care Coordinator is responsible for managing the daily meeting when the manager is unavailable, purchasing supplies, and managing the day-to-day operations. This includes managing the schedule of the Psychiatric ARNPs and assisting with the scheduling of medication delivery.

Responsibilities:
  • Work collaboratively with key stakeholders and partners of PACT and CLR.
  • Manage the front-end referral process. Including collecting patient collateral information and reviewing EPIC data.
  • Manage discretionary funds used by staff for consumers.
  • Ensure the company vehicles are regularly maintained.
  • Aid in discharge planning, which may include working with the MCO's and providing solid outcome data to funders.
  • Purchase office supplies and consumer food items for group activities.
  • Perform as a supportive member of committees and project meetings, in coordination with agency departments to ensure agency and program compliance with applicable reporting requirements and regulatory mandates to ensure that administrative operations follow these requirements.
  • Provide support to clinical programs, human resources, information services, and other operational areas to coordinate the implementation and sustenance of the agency quality improvement and key performance indicators that monitor progress in reaching annual outcome measurement goals.
  • Participate in University of WA spirit lab trainings and comply with fidelity standards.
  • Maintain current knowledge and participate in the development of policies and procedures, contracts, and other regulations and requirements, including WACs, Joint Commission, SAMHSA, and CCBHC guidelines.
  • Participate in making recommendations for improvements and solutions that enhance the quality of agency programming and processes for data collection workflows.
  • Perform other duties as assigned.
Qualifications:
  • Bachelor's degree or relative experience preferred.
  • Strong organizational skills.
  • Must have excellent customer service skills.
  • Experience in non-profit or healthcare organizations is preferred.
  • Strong skills in Microsoft office applications.
  • Valid Washington State driver's license; proof of insurance; provide own transportation.
