Job Summary

The Stroke Program Coordinator/Quality Educator is the clinical leader for the organization of services and systems necessary for a collaborative approach to stroke care and assigned unit based educational needs. The advanced knowledge and skills required for this role include clinical expertise in a focus area, evidence-based practice, collaboration, consultation, education, mentoring, and change leadership. Accountable for collaborating with members of the health care team to design, implement, and measure safe, cost-effective, evidence-based care strategies. Stroke Program Coordinator/Quality Educator will assume the day-to-day responsibilities of process and performance improvement activities as they relate to nursing, and ancillary personnel and assist in carrying out the same functions for all staff.

Core Duties:
  • Plans, implements and evaluates clinical programs to achieve high quality, patient-focused outcomes which advance patient and family care and nursing practice in stroke care.
  • Responsible for the Stroke Program development, quality assessment, improvement and inter/intra disciplinary communications and will work collaboratively with all hospital and clinic personnel on shared system and performance improvement projects.
  • Serves as a clinical resource for physicians, nurses, and clinical staff participating in the care of stroke patients by providing stroke education based on best practice guidelines.
  • Responsible for collecting, storing and reporting data collection and for quality improvement of the stroke program to include chart abstraction and entry into Get with the Guidelines using Joint Commission and CMS specifications with adherence to regulatory deadlines.
  • Responsible for creating and implementing appropriate action plans based on identified clinical gaps.
  • Responsible for maintaining current professional knowledge and competencies and contributing to the advancement of the practice of nursing at the unit/system, local, state and/or national level.
  • Utilizes adult learning principles, teaching and learning theories, and an understanding of generational learning styles to identify appropriate teaching strategies in planning educational programming.
  • Collaborates with interdisciplinary teams, leaders and others to strengthen nursing practice, facilitate change and generate positive outcomes.
  • Plans, implements and evaluates unit based clinical education programs for clinical staff.
  • Accountable for collaborating with members of the health care team to design, implement, and measure safe, cost-effective, evidence-based care strategies
  • Responsible for clinical compliance according to The Joint Commission regulatory guidelines for Stroke Center certification and ensuring readiness for The Joint Commission stroke survey.
Other Duties:

Minimum Qualifications:
  • Master's Degree preferred
  • BSN and currently enrolled in Master's Degree program
  • RN licensure in RI
  • Current valid BLS certification from the American Heart Association
  • 1 year experience in nursing education
  • 5 years of clinical experience
Required Certifications, Licensure:
  • National professional specialty certification preferred
