Job DescriptionJoin the CentraCare Senior Transitions team as an Outreach Coordinator.The CentraCare Senior Transitions team offers whole-person primary care in assisted living communities, transitional care units, memory care, and long-term care settings. The goal of our provider and nursing teams is to care for patients in whatever senior community they call home. Currently, our team sees patients or will begin seeing patients in St Cloud, Monticello, St Joseph, Cold Spring, Watkins, Paynesville Belgrade, Long Prairie, Melrose and Sauk Centre. The Senior Transitions Outreach Coordinator serves as the liaison between CentraCare's Senior Transitions team and the assisted living, nursing home, and transitional care facilities where the Senior Transitions team provide care. The Outreach Coordinator will enhance communication, identify educational opportunities, and improve quality outcomes of services. In addition, will maintain a regular presence at all Senior Transitions sites to enable informal communication, provide real-time education, and build relationships with assisted living, nursing, and transitional care facility staff, and leadership. Solicits feedback from staff to identify areas that the Senior Transitions service could augment. Serves as a resource for all sites of service as well as use of CentraCare's EpicCare Link program. Monitors patient care at sites to identify opportunities for improvement. Works to develop and implement those interventions along with Senior Transitions leadership. Coordinates, facilitates, and participates in scheduled outreach events at sites of service related to staff education, patient education, patient onboarding, and generating clinic panel growth. Maintain ongoing communication and a consistent follow-up with all sites of service and Senior Transitions team members regarding potential patients and families who have yet to commit to service. Responsible for the implementation of marketing and communication plan. Analyze program performance reports and make recommendations designed to improve or enhance clinic performance. Provide onboarding support for new patients and their families as well as for the site clinical teams and the Senior Transitions team members. Coordinate with the CentraCare IS team to ensure problems and system development needs are addressed related to use of EpicCare Link system. Provide organized feedback regarding operational issues and administrative matters to Director of Senior Transitions and make recommendations to facilitate further program development.Schedule: Full Time | 72 hours per 2 weeks Monday - Friday 8 a.m. to 4:30 p.m. Will need to travel to locations where Senior Transitions provides servicesPay and Benefits: Pay starting at $20.45 per hour however, we pay more for experience.
