CentraCare's Senior Transitions team is dedicated to providing holistic primary care to patients residing in senior communities, facilitating their journey back home following stays in transitional care units. Our services encompass routine primary care, management of acute illnesses, and support for chronic condition management. Furthermore, we ensure the coordination of essential medical services and equipment for our patients.
Key Responsibilities:
- Prepare for healthcare provider appointments by compiling patient records and assisting with patient intake in the Epic system.
- Address follow-up requirements identified by healthcare providers after patient visits.
- Facilitate communication between nursing staff at the site and the Senior Transitions team.
- Manage prescription prior authorization processes as necessary.
- Collaborate with other healthcare specialists involved in patient care.
- Utilize a hybrid work model, alternating between in-person visits and remote work.
- This is a full-time role requiring 72 hours of work every two weeks.
- Daytime shifts are scheduled between 7 AM and 5 PM, with no weekend or holiday shifts.
- A hiring bonus of $2,500 is available.
- Starting hourly wage is $19.66.
- Comprehensive benefits package includes medical, dental, paid time off, retirement plans, and more.
- Must be a graduate of an accredited nursing program.
- Current licensure as a Licensed Practical Nurse in Minnesota is required.
- AHA BLS Healthcare Provider certification is necessary.
- A valid Minnesota driver's license and dependable transportation are required.
