Overview:

To coordinate assessment of each residents condition at admission, quarterly, annually, and when significant and permanent changes in residents condition occur; to coordinate completion of PPS assessments on Medicare A residents; to assist in the development of a comprehensive care plan to address the needs of each resident.

Responsibilities:
  1. Assist with screening of residents prior to admission for appropriate placement.
  2. Conduct admission conference with resident and family to address any resident/family concerns and to review facility policies.
  3. Monitor residents (Part A&B) condition on a regular, ongoing basis and assure accurate, complete documentation is available in the Medical Records.
  4. Perform PPS assessments at the required intervals as determined by the Medicare program. Assure completion of comprehensive assessment within 14 days of admission, annually, quarterly, and when significant changes occur in residents condition.
  5. Assist Social Services with setting up interdisciplinary care plan meetings with resident/family at regular intervals to discuss resident needs and discharge planning.
  6. Assure development of care plans to meet the current needs of the resident with input from resident and family and in conjunction with the interdisciplinary care planning committee.
  7. Track Part A & B case mix weekly.
  8. Maintain distinct part census with appropriate RUGS.
  9. Cooperate with the Rehab Department in determining appropriate RUG classification and utilization of resources.
  10. Conduct Rehab meetings weekly.
  11. Transmit assessments within regulated time frame.
  12. Support this facilitys commitment in the quality of services to residents, family members, and the public.
  13. Be able to hear, speak and see.
  14. Be able to be on feet for several hours while bending, stooping, stretching, and lifting to assist residents.
  15. Hold confidential all company, facility, resident, or employees information.
  16. Report all incidents/accidents immediately to supervisor.
  17. Attendance at all required in-service training programs and staff meetings.
  18. Adhere to facility-established policies and procedures.
  19. Perform other duties as assigned or required.


Qualifications:

Must be a Registered Nurse or a Licensed Practical Nurse currently licensed in the state in which the facility is located. In performing the essential functions of this job, with or without reasonable accommodations, the individual cannot pose a direct threat to property, themselves, or others. Regular attendance must be maintained by the individual holding this position.
