Job Summary

We are seeking a highly skilled and compassionate MDS Coordinator/Patient Care Manager to join our team at Ball HealthCare. As a key member of our healthcare team, you will be responsible for coordinating and documenting assessments of residents, developing comprehensive care plans, and ensuring that individual resident care plans are being followed and updated as necessary.

Key Responsibilities
  • Assessment and Care Planning: Coordinate and document assessments of residents to identify their healthcare needs and develop comprehensive care plans that meet those needs.
  • Comprehensive Care Planning: Assist in the development of comprehensive care plans in accordance with facility, state, and federal guidelines.
  • Resident Monitoring: Monitor residents to ensure that individual resident care plans are being followed and updated as necessary.
Requirements
  • Education and Licensure: Must be a Registered or Licensed Nurse in good standing and currently licensed by the state of Alabama.
  • Experience: Must have at least 1 year of experience in MDS and geriatric care.
