Treatment services are delivered at flexible times that meet clients' needs and in settings that offer the best potential for positive outcomes, including in hospital settings, in-home, and in community-based settings.Provide strengths-based, client-centered, and trauma-informed services to adults with severe mental illness, history of homelessness, and co-occurring substance use disorders. Provide program orientation, monitor staff documentation, complete payroll and co-lead team meetings. Coordinate with manager and director to ensure grant deliverables are met.Responsibilities Provide day-to-day support and coordination to team members and oversee schedule to ensure service coverage across Front Door caseload Provide direct service activities through coverage, crisis intervention, and providing housing and case management services to a small caseload of 2-3 clients Coordinate with Front Door leadership to determine capacity for potential diversions, coverage for day-of-diversion activities and 24-hours check-in, and ensure comprehensive treatment services are available Provide in-person and telephone services to clients and treatment teams to coordinate care and resolve client concerns Meet regularly with KYC treatment teams to integrate care provision Communicate team expectations regarding dignity of risk, harm reduction and housing first philosophies, and clinically appropriate linkage and care coordination Ensure that all services and clinical activities provided by all team members are compliant with standards, policies and procedures of the organization and clinical best practices Ensure entire team completes documentation and charting in compliance with grant requirements, state specific accreditation, and agency standards Ensure team is meeting productivity requirements and grant deliverables Identify potential NIH (No-Income Housing) referrals and ensure connection to NIH team Participate in clinical and administrative supervision as directed, to receive and offer feedback and for performance evaluation; seek additional clinical and program consultation as needed and alert Manager, Deputy and Director leadership of potential clinical and program needs Coordinate with HR, Information Systems, and Facilities to ensure adequate training and operations of the team Participate in interviewing and hiring and provide initial program orientation to new employees; oversee day to day clinical and administrative operations and activities Provide weekly and ad-hoc clinical and administrative supervision to a team of 4-8 direct reports, including clinical direction and administration with treatment planning and delivery Complete regular co-visits with staff to include modeling and observing services provided as part of training, quality assurance, and assessments and crisis intervention support Provide immediate crisis intervention and de-escalation to support clients and team members during business hours as needed; coordinate as needed with 590 Crisis team and Living Room Provide performance evaluations for all supervisees, building on strengths and identifying ongoing growth/training needs, and supporting professional development Lead team meetings Provide rotating support on weekends as clinical consult for assessment team, approximately once per month (for QMHP staff only) Submit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Center's policies and procedures Education and Experience Meet Illinois DMH Rule 132/140 guidelines for MHP (Mental Health Professional) criteria Preferred Bachelor's or Master's Degree from an accredited college or university in social work, psychology, or counseling. 3 years experience and extensive knowledge of case management and housing services 2 years of experience working with adults with serious mental illness Preferred previous supervisory or leadership experience Preferred experience working with homeless populations and/or co-occurring substance use Preferred experience working in non-traditional treatment environments i.e.in-home, residential, hospital, and community settings Applicant must have a valid IL Driver's License and daily access to a well maintained/insured vehicle Basic computer and smartphone proficiency, knowledge of Word, Excel, and internet skills, team collaboration tools, and emailWork Environment / Schedule While performing the administrative duties of this job, the employee regularly works in an office setting at least 50% of the time with standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines Up to 25% of the employee's time will be spent in the community, driving to and from and providing services in hospital settings and client homes In supporting clients to live independently, employee may be exposed to varying home conditions Position is located primarily in KYC office (Elk Grove/Arlington Heights). After orientation and training period, option for hybrid to allow remote work-from-home 1-2 days per week depending on program and team needs. Coverage must be coordinated with other program leadership and approved by DirectorSchedule Non-Exempt, Full-Time, 37.5 hour work week Position is primarily Monday-Friday during business hours.
