•Ensures member access to services appropriate to their health needs.

•Conducts screenings and gathers information to identify individual needs for addition to the care management plan.

•Coordinates internal and external resources to meet identified needs.

•Monitors effectiveness of the care management treatment plan and escalates concerns to RN, SW, or Provider teams.

•Interfaces with the clinical care team and community partners in the development of care management plans.

•Makes home visits to patients at place of residence or other facility (e.g. homeless shelter, assisted living, skilled nursing facility).

•Arranges for transportation to provider visits if needed.

•Coordinates community resources to address gaps in care; food, housing, support, etc.

•Engage with patients as directed to identify social determinants of health.

•Comfortable working with patient that may have mental illness and substance abuse issues.

•Make follow up phone calls as needed.

•Ability to work independently under the direction of Population Health and Care Management staff.

Required Qualifications:
  • Associate's Degree Health Related, Community Outreach or Social work related field. Or equivalent educ/experience
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 1 year Health Related, Community Outreach or Education Experience
Preferred Qualifications:
  • Bachelor's Degree Health Related, Community Outreach or Social Work Related Field Or equivalent educ/experience
  • Preference will be given to applicants with lived experience

About the Team

Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
