Job Summary

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Patient Health Navigator to join our team. As a Patient Health Navigator, you will play a vital role in ensuring that patients receive high-quality, patient-centered care. Your primary responsibility will be to navigate patients through the healthcare system, providing support and guidance to help them access the care they need.

Key Responsibilities
  • Schedule Appointments and Coordinate Care

Work closely with patients, healthcare providers, and other stakeholders to schedule appointments and coordinate care. Ensure that patients receive timely and effective care, and that their needs are met.

Provide Patient Support and Education

Offer patients support and education to help them manage their health conditions, navigate the healthcare system, and make informed decisions about their care. Provide information and resources to help patients understand their health status and treatment options.

Collaborate with Healthcare Teams

Work collaboratively with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure that patients receive comprehensive and coordinated care. Participate in multidisciplinary teams to discuss patient care and develop plans to address patient needs.

Identify and Address Health Disparities

Identify and address health disparities and barriers to care that may impact patient outcomes. Work with patients, families, and communities to develop strategies to overcome these barriers and improve health outcomes.

Develop and Implement Care Plans

Develop and implement care plans that are tailored to the unique needs of each patient. Work with patients, families, and healthcare teams to develop and implement plans that address patient needs and promote optimal health outcomes.

Requirements
  • Education and Experience

A high school diploma or equivalent is required. A degree in a healthcare-related field, such as nursing or health education, is preferred. At least one year of experience in a healthcare setting is required.

Communication and Interpersonal Skills

Excellent communication and interpersonal skills are required. Ability to work effectively with patients, families, and healthcare teams is essential.

Problem-Solving and Critical Thinking

Ability to analyze complex patient situations and develop effective solutions is required. Strong critical thinking skills are essential for this role.

Cultural Competence

Cultural competence is essential for this role. Ability to work effectively with diverse patient populations and to understand and address cultural and linguistic barriers to care is required.

Language Skills

English language proficiency is required. Bilingual or multilingual skills are preferred.
