Job Summary

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Patient Health Navigator to join our team. As a Patient Health Navigator, you will play a vital role in ensuring that patients receive high-quality, patient-centered care. Your primary responsibility will be to navigate patients through the healthcare system, providing them with the necessary support and resources to achieve optimal health outcomes.

Key Responsibilities
  • Schedule Appointments and Coordinate Care

Work closely with patients, healthcare providers, and other stakeholders to schedule appointments and coordinate care. Ensure that patients receive timely and effective treatment for their medical conditions.

Provide Patient Education and Support

Develop and implement patient education programs to promote healthy behaviors, disease prevention, and management. Provide emotional support and guidance to patients and their families, helping them to navigate the healthcare system and access necessary resources.

Collaborate with Healthcare Teams

Work closely with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure that patients receive comprehensive and coordinated care. Participate in multidisciplinary teams to develop and implement care plans that meet the unique needs of each patient.

Identify and Address Health Disparities

Identify and address health disparities and barriers to care, working to ensure that all patients have equal access to high-quality healthcare services. Develop and implement strategies to address social determinants of health, such as housing, food insecurity, and transportation.

Document Patient Information and Outcomes

Maintain accurate and up-to-date patient records, documenting patient information, treatment plans, and outcomes. Use data to track patient progress and identify areas for improvement.

Requirements
  • Education and Experience

A high school diploma or equivalent is required. A degree in a healthcare-related field, such as nursing or health education, is preferred. At least one year of experience in a healthcare setting, preferably in a patient navigation or care coordination role.

Communication and Interpersonal Skills

Excellent communication and interpersonal skills are essential for this role. Ability to work effectively with patients, healthcare providers, and other stakeholders to achieve common goals.

Organizational and Time Management Skills

Strong organizational and time management skills are required to manage multiple patients and prioritize tasks effectively.

Cultural Competence and Sensitivity

Cultural competence and sensitivity are essential for working with diverse patient populations. Ability to work effectively with patients from diverse backgrounds and to adapt to changing cultural and linguistic needs.

Language Skills

Fluency in English and Spanish is preferred, but not required. Ability to communicate effectively with patients and healthcare providers in their preferred language.
