We are seeking a skilled Social Worker to join our team at MultiCare Health System in Covington, Washington.
Key Responsibilities- Provide cross continuum care activities, including clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care, as appropriate.
- Offer customer service to patients, families, physicians, and care team members, ensuring seamless transitions of care.
- Model and orchestrate consistent, professional communications and maintain proactive communication patterns with colleagues and staff.
- Delegate responsibilities appropriately while remaining accountable for outcomes.
- Master's degree in Social Work with a healthcare-based practicum.
- Licensed in the state of Washington as a Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
- Hospital or equivalent experience preferred; experience prior to obtaining a Master's degree may be considered.
- Health industry knowledge and healthcare financial assistance knowledge preferred.
